TYMS Transition to Adulthood

Our TYMS Transition to Adulthood Support Workers guide our young people to look towards the future and support the development of life skills i.e.
  • Identifying their strengths/interests to help them achieve their goals
  • Identifying/accessing adult services and helping them become confident in doing this independently
  • connecting them to their identity, culture, whanau and wider networks or support.​

​We provide our Transition Support services in partnership with Oranga Tamariki. 
Our Aim 
Who is the service for?
The purpose of our transition to adulthood service is to see the following differences being made for our young people
  • more young people have safe and stable living arrangements
  • more young people have the life skills they need to thrive as adults
  • more young people are healthy and recovering from trauma
  • more young people are in education, employment or volunteering
  • reduced disparities in outcomes and experiences for Pasifika, tamariki Maori,  and their whanau 
  • children and young people feel more listened to and understood
To be eligible, a rangatahi will have been in one or more of the following, for a continuous period of at least three months after the age of 14 years and 9 months:
  • a care and protection placement
  • a residential youth justice placement (including remand) or Police custody; or
  • under remand or a prison sentence in the adult justice system before turning 18.
Most young people referred to the Service will be aged 16 and 17. Exceptions may be made for care-experienced younger people aged 15 who have made the decision to live independently (and have been deemed capable to do so).

Young adults older than 21 years are entitled to advice and assistance up to the of 25.
